    The following questions will enable us to create your account so you don't need to when you arrive. These questions are legally required in the state of PA for all who UV tan. Please note only children aged 17 are able to UV tan and parent must come in 1st visit to authorize underage tanning. 16 and under can red light, massage or spray tan. 

    Our goal is to always educate, prevent burning and build a smart tan. The following questions will help us help you better! Since our main business is UV, we require all clients to fill this out even their intentions at first are to not tan and use other services. 

    We ask this because you may not be able to get a tan from our units if you've never been able to tan outdoors.
    We ask this because if you're at risk for burning we will put you in a bed with less uvb and start you slower so you can build melanin without burning.
    We take this into consideration for when we set up a tanning routine for you.
    Medications that increase sensitivity: Antibiotics, particularly tetracyclines like doxycycline and fluoroquinolones like ciprofloxacin Tricyclic antidepressants like amitriptyline and nortriptyline Older antihistamines like promethazine Griseofulvin, an antifungal medication Quinine and other antimalarial medications Acne medications like Accutane (isotretinoin) and Retin-A (tretinoin) Methotrexate, which is used for certain autoimmune conditions and cancers Sulfonylurea drugs for diabetes like glyburide, glipizide, and glimepiride Hydrochlorothiazide (HCTZ) and other thiazide diuretics Some heart medications for arrhythmia, including amiodarone, which can cause a blue-gray discoloration of the skin.     We always suggest talking to your Doctor before tanning while pregnant.
    You are required by PA to wear eye protection while using a UV device. Sanitize your eyewear after each session and replace after 3 months. 1 session without protection and cause irreversible eye damage.
    It is recommend to remove contact lenses prior to a uv session.
